• "Qualification Form" refers to any form, application, inquiry, quote request, or submission made through the Websites requesting connection with Providers.

Modifications

Coverage Care Group reserves the right to modify the Websites or this Agreement at any time. Changes are effective immediately upon posting. Continued use constitutes acceptance.

Privacy Policy

Your use of the Websites is governed by our Privacy Policy, incorporated herein by reference.

Insurance Request & Matching Services

Coverage Care Group is a licensed independent insurance agency operating under applicable state licenses. We do not underwrite insurance policies.

We may:

• Collect information from your Qualification Form
• Submit that information to one or more Providers
• Match you with licensed insurance agents or carriers
• Receive compensation from Providers

We do not guarantee:

• Acceptance into any insurance program
• Approval by any carrier
• Specific pricing or policy terms
• That any Provider will contact you
• That any rate offered is the lowest available

Approval standards and underwriting decisions are determined solely by the Provider. Policies may not be available in all states.

Medicare & Government Plan Disclosures

If you request Medicare-related information, a licensed insurance agent may contact you to confirm eligibility and discuss plan options. Availability and eligibility are determined by the issuing carrier and applicable regulations.

We do not represent the federal government.

Express Written Consent to Contact (TCPA)

By submitting any Qualification Form and electronically signing or clicking submit, you provide express written consent for:

• Coverage Care Group
• Our affiliates
• Service providers
• Matched Providers

To contact you via:

• Phone calls
• SMS / text messages
• Artificial or prerecorded voice
• Automatic telephone dialing system

Even if your number is listed on any federal, state, or internal Do-Not-Call list.

Message and data rates may apply. Consent is not required as a condition of purchasing insurance.

You may revoke consent by:

• Replying STOP to SMS
• Contacting us directly at admin@coveragecaregroup.com
• Notifying a representative verbally

You must contact Providers directly to stop communications from them.

Email Consent

You consent to receive marketing and transactional emails from Coverage Care Group and matched Providers.

You may unsubscribe via the link in the email.

SMS Messaging Terms & Conditions

By providing your mobile phone number and opting in, you agree to receive SMS text messages from Coverage Care Group LLC.

Program Description

Coverage Care Group LLC may send text messages regarding:

• Insurance consultations
• Quote follow-ups
• Appointment scheduling and reminders
• Policy updates
• Customer service communications

Message Frequency

Message frequency varies but typically will not exceed 4 messages per month, unless additional messages are required to fulfill your request.

Cost Disclosure

Message and data rates may apply based on your mobile carrier plan.

SMS Opt-Out Instructions

You may cancel SMS communications at any time by replying:

• HELP for assistance

After you send STOP, you will receive a confirmation message and no further messages will be sent unless you opt back in.

Carrier Liability Disclaimer

Carriers are not liable for delayed or undelivered messages.

Eligibility

By opting in, you confirm that you are the authorized user of the mobile number provided.

Data Retention & Regulatory Compliance

Due to insurance, regulatory, and anti-fraud laws, your Personally Identifiable Information (PII):

• May be retained as required by state or federal law
• May not be deleted until statutory retention periods expire
• May be shared with Providers to meet regulatory obligations

You provide express consent for this retention and sharing.

Identity Verification & AML

To help fight identity theft, fraud, terrorism financing, and money laundering, Coverage Care Group and Providers may obtain, verify, and record information including:

• Social Security number
• Address
• Government-issued identification

We may be examined by regulators and are required to retain certain information.

Compensation Disclosure

Coverage Care Group receives compensation from Providers for:

• Lead generation
• Policy placement
• Commission payments
• Marketing fees

Your use of the Websites constitutes agreement to this compensation arrangement.

Indemnification

You agree to indemnify and hold harmless Coverage Care Group and its officers, employees, affiliates, licensors, and Providers from any claims, losses, damages, or expenses (including attorneys' fees) arising from:

• Your violation of this Agreement
• Your misuse of the Websites
• Your violation of any law or third-party right

Errors & Delays

We are not responsible for errors or delays caused by:

• Incorrect information provided by you
• Technical issues beyond our control
• Carrier processing delays

Third-Party Links

We are not responsible for third-party websites linked from our Websites.

Dispute Resolution & Arbitration

You agree that any dispute shall be resolved by binding individual arbitration administered by the American Arbitration Association (AAA) under its Consumer Rules.

Arbitration shall occur in Palm Beach County, Florida.

You waive:

• Jury trial
• Class actions
• Consolidated claims

All arbitration must be conducted individually. Any dispute must be filed within one (1) year from the date it accrued.

Florida law governs this Agreement. If any matter proceeds in court for enforcement of arbitration, exclusive venue shall be Palm Beach County, Florida.

Account Responsibility

If you create an account:

• You must provide accurate information
• You are responsible for safeguarding credentials
• You are responsible for all activity under your account

Notify us immediately of unauthorized use.

Services Jurisdiction

Services may only be available in states where Coverage Care Group or the Provider is licensed.

Provider participation does not constitute an offer outside authorized jurisdictions.

Entire Agreement

This Agreement constitutes the entire agreement between you and Coverage Care Group. If any provision is deemed unenforceable, the remainder remains in effect.
